PREADMISSION
IMMUNIZATION MEDICAL POLICY
- Purpose:
To establish procedures
that require all students to present immunization documentation
and complete mandatory health records as a prerequisite
for registration.
- General
: To establish guidelines
for Newberry College to promote wellness on campus and within
the dormitories by preventing the spread of disease and
to provide health and wellness to all students in a confidential
manner.
- Procedures:
- All
students are required to complete a Medical History Form
and a Student Immunization Record. A copy of the immunization
record or the student's health care provider must provide
the appropriate information indicating immunity to Measles,
Mumps, and Rubella. A tuberculin skin test must be completed
within 12 months prior to admission. All blanks and signatures
must be completed and signed on the medical history form.

- All
health information is required to be on file at the Wellness
Center .
- This
policy will be included in the Student Handbook and will
be placed on the Wellness Services web site.
- The
mandatory information will be provided to parents at orientation
by the Director of Wellness Services.
- IF
THE STUDENT FAILS TO COMPLY WITH SUBMITTING THE REQUIRED
INFORMATION, THE STUDENT WILL NOT BE ALLOWED TO REGISTER
FOR CLASSES!
