PREADMISSION IMMUNIZATION MEDICAL POLICY

 

  1. Purpose: To establish procedures that require all students to present immunization documentation and complete mandatory health records as a prerequisite for registration.
  2. General : To establish guidelines for Newberry College to promote wellness on campus and within the dormitories by preventing the spread of disease and to provide health and wellness to all students in a confidential manner.
  3. Procedures:
    • All students are required to complete a Medical History Form and a Student Immunization Record. A copy of the immunization record or the student's health care provider must provide the appropriate information indicating immunity to Measles, Mumps, and Rubella. A tuberculin skin test must be completed within 12 months prior to admission. All blanks and signatures must be completed and signed on the medical history form. See attached medical history and immunization form.
    • All health information is required to be on file at the Wellness Center .
    • This policy will be included in the Student Handbook and will be placed on the Wellness Services web site.
    • The mandatory information will be provided to parents at orientation by the Director of Wellness Services.
    • IF THE STUDENT FAILS TO COMPLY WITH SUBMITTING THE REQUIRED INFORMATION, THE STUDENT WILL NOT BE ALLOWED TO REGISTER FOR CLASSES!
